Tips for Completing Your Forms
- Use a separate form for each child.
- Fill out all sections highlighted in blue and white.
- Go over any changes with your center director.
- Bring your enrollment agreement and your driver’s license or ID so your center director can check your information.
If any of your information changes, meet with your center director. They’ll go over the updates, confirm your tuition rate, sign the agreement, and save a copy in your family file.
